What is meant by Hair Transplant?

Hair Transplant is a surgical procedure where hair follicles are transplanted from one part of the body to the scalp or face of a patient. The part of the body from which the follicles are taken is called the donor site and the part of the scalp or face where follicles are placed is called the recipient site.

Remember, the hair transplantation is a process that starts with the decision of the person to have hair transplantation and completely ends about 1 year after the procedure.  For this reason, it is important that you work with reliable companies that will be with you in the whole process.

 

Is Hair Transplant permanent?

Hair transplants are usually permanent, as the translanted follicles are taken from areas where hair loss will not occur. Patients may experience hair loss in other areas of their scalp or face but in not the recipient site. The transplanted hair will fall out after the surgery, but will grow back within about six months. Surgeons usually prescribe medications to strengthen hair follicles to prevent the loss of non-transplanted hair after the surgery.

 

What are the different types of Hair Transplant?

Two types of hair transplantation techniques predominate, including the follicular unit transplantation (FUT) and follicular unit extraction (FUE) techniques.

In FUT, the grafts come from the strip of skin removed. But in Follicular Unit Extraction, the follicular grafts are extracted individually from the donor area and then transplanted. The minimally invasive FUE method where a hole is made in the donor site with a punch and single hair follicles are taken from the donor site and placed in the recipient site.  Since the FUT technique leaves scars, it is not possible to achieve an aesthetically natural appearance. We prefer the FUE method for our patients, in the table below we have prepared for you, you can find the important differences between the FUE and FUT method. 

The duration of the operation performed with the FUE technique varies between 4-8 hours, depending on the number of grafts. Since local anesthesia is applied before the procedure, no pain is felt during the operation.

The person can listen to music, watch television, read newspapers and books until the end of the hair transplant.

The first few weeks and especially the first 3-7 days after hair transplantation are very important for successful results in hair transplantation.

In this process, the person should pay attention to his head and avoid possible blows and contacts. He should wear a hat or bandana when he goes out, and stay away from unhygienic environments and external factors such as sun and rain that will come into direct contact with the hair. Particular attention should be paid to the sleeping position for the first 3 days. You should lie on your back and the hair transplant area should not come into contact with the pillow.

The transplanted hair may fall out in the first few months after the operation. This is an expected situation. Sometimes it can be poured early, sometimes it can be poured late, sometimes even not at all. It varies from person to person depending on the physiological condition. The first hair starts to grow about 3 months after hair transplantation. At the end of the 7th month, most of the transplanted hair will come out. At the end of the 1st year, all of the transplanted hair will come out.

Can women have hair transplant?

Of course, just like men, women can also have hair transplants. The causes of hair loss should be investigated before the transplantation, and if there is a hormonal disorder or similar problem, it should be treated before the transplantation. Hair transplantation should be done after the underlying cause is treated.

What is the age limit for hair transplantation?

Hair transplantation can be successfully applied to anyone over the age of 18 with male pattern hair loss. If you have an important disease, allergy or a condition such as blood pressure, diabetes, heart, you should definitely tell your doctor before the procedure. In such cases, it is decided whether you are a suitable candidate for hair transplantation during the pre-controls.

Is hair transplantation a painful procedure?

Hair transplant operation is a procedure performed with local anesthesia. The person only feels the procedure while applying local anesthesia. Since the area is numb after local anesthesia, no pain is felt. Pain may occur after the operation. These can be prevented with recommended painkillers.

1 - Hair transplantation to an unsuitable person

  • Age:

If there is hair loss at a very young age and rapidly, it is not correct to decide on hair transplantation, the reasons should be well investigated.

  • Family Hair Loss Story:

The type and speed of hair loss in first degree relatives such as father, older brother and uncle should be questioned. For example, a father who is Norwood 7 will likely have his son in the same situation as the father. This should be clearly stated to the person who is considering a hair transplant.

  • Hair Loss Type and Speed:

In patients with widespread hair loss such as Norwood 6-7, the hair follicles taken may not be enough to cover the bald area.

  • Donor Area status and Donor Capacity:

It is an important criterion.

How many hair follicles can be taken from the donor area? How many square centimeters is the balding area?

How many roots are planned to be placed in one centimeter?

These should be calculated before hair transplantation. A person who is taken to the operation randomly without calculation is also a candidate for a bad hair transplant result.

3- Mistakes Made in Graft Extraction

 

In applications made by people who are not experts in their work, mistakes related to wrong rooting are frequently made. Some of the common mistakes are;

  • Taking more grafts than necessary:

Before hair transplantation, you should definitely be examined by a doctor.

The estimated number of grafts that can be taken in accordance with your hair and root structure is calculated and your treatment is planned.

Everyone has a donor capacity. If a large number of grafts are taken by forcing the donor capacity to remove excess hair follicles, unpleasant images will occur a few months after hair transplantation.

The number of grafts that can be taken varies from person to person. Approximately 20-25% of the number of roots per square centimeter should be taken. A larger number of grafts will distort the image in the extraction area.

  • Failure to take grafts homogeneously:

An equal number of grafts should be taken from each centimeter. After inhomogeneous root extraction, moth-hole (piece-meal) images may occur.

  • Less grafts should be taken from the above-ear areas.

The hair follicles on the ear are less in number and thinner than the middle regions. If roots are taken excessively from the above-ear areas, serious cavities and appearance disorders may occur.

  • Improper use of micromotor or manual tools:

The angles and lengths of the roots should be determined correctly when taking manual or micro-motor extraction. If roots are not taken at the appropriate angle and depth, some of the hair follicles will be traumatized and broken or crushed. Therefore, the roots taken and transplanted in this way will not grow, and it will be considered as a bad hair transplant since there will be no expected number of grafts.

  • Micro motor speed:

The majority of purchases in the donor area are made with a micro motor. Unfortunately, in places where a large number of transplants are made in one day, it is essential to take root quickly and perform an operation on another patient. In order to take root quickly, it is necessary to increase the micro motor speed.

High speed means heat increase. In this way, the roots will be destroyed due to high temperature while being taken. Therefore, it is normal to receive 1000-1200 grafts per hour. Instead of taking 4000 grafts in 1 hour, it is right to remove all the grafts intact.

  • Punch Width:

In hair transplantation, it is not correct to take root with very wide or very thin punches. Everyone's hair follicle thickness is different. Punchs should be selected with a width suitable for the patient's hair root. Punch widths of 0.7, 0.8 and 0.9 mm are ideal. If very large punches are taken, serious scars are seen in the donor area. If thick grafts are taken with very thin punches, the incision rate will increase in the roots we call transection. This means graft loss.

  • Number of Roots in a Session:

One of the common causes of unsuccessful hair transplantation is that the hair transplantation period is too long. The longer the time, the longer the roots will remain outside. This will reduce the rate of the roots to remain intact, causing fewer hair follicles to emerge than expected.

4- Mistakes Made During Hair Transplantation

  • Wrong Hairline design and planting roots at wrong angles:

It is one of the the most common cause of bad hair transplant complaints. Single and thin grafts should be transplanted into anterior grafts. It is important to make a design that looks as natural as possible instead of the so-called "grass man" image. For this reason, the person who will design your hair transplant should be experienced.

 

  • Depth of Planted Roots:

Roots planted too deep are always a problem. Hair does not grow from most of the deeply planted roots. In deeply planted and hair-growing roots, it causes some problems scars, hair root inflammations like folliculitis and curly hair growth.

  • Very Frequent Hair Transplantation:

Patients want the hair to be very dense. This is a perfectly natural request. Between 30-40 grafts per square centimeter in 1 session is ideal. It is a mistake to try to achieve a much higher density in 1 session. The vascular structure may be severely damaged. Hair follicles need nourishment through capillary vascular structure to survive. Therefore, a large part of the roots whose nutrition is reduced will not grow and will be considered as bad hair transplantation. Already 30-40 grafts planted per centimeter will create an ideal density. If the person wants a more frequent view, it is more correct to plan a second session after 1 year the first hair transplant.

  • Very Rare Hair Transplantation:

If the person's expectation is very high, it is best not to transplant hair in large areas. Let's explain with an example. Suppose a person with Norwwod 7 and 3000 grafts of hair grafts are taken. The balding area of a person with Norwood 7 hair loss type is at least 300-400 square centimeters. If we spread 3000 grafts over the entire balding area, there will be 10 hairs per square meter. This will create a very sparse image. The person will certainly not be happy with this result. It would be more accurate to increase the number of roots per square centimeter by planning planting for a narrower area. Please be sure that everything is well planned before the procedure and that your doctor has examined you well.

  • Tissue Necrosis and Infections:

Necrosis after hair transplantation is one of the most frightening complications.

Due to the unconscious use of adrenaline, a partial area cannot be fed and dead tissue, which we call tissue necrosis, occurs there. Generally, a round or oval shape of 2-3 cm in diameter appears without hair. In addition, infections may occur if sterile work is not performed during hair transplantation. Inflammation occurs in the hair follicles. It seriously reduces the rate of hair growth. For this reason, please make sure that you are working with experienced clinics or consultants who are headed by physicians and have been doing this job for a long time.

5-Mistakes Made After Hair Transplantation

​

Mostly, these mistakes are patient’s faults. No one wants the result of a hair transplant to fail. They are usually unintentional mistakes. The most critical period after hair transplantation is the first 1 week. In particular, a blow to the head may cause bleeding to the planting area and dislodgment of the roots. This situation can be compensated by replanting the displaced roots later.

The person who has a hair transplant should follow some simple recommendations. For example, the hair transplant area should be protected from sunlight for the first month. It should not be exposed to heat like a bath or sauna, as this may damage the roots.
